In consideration of being permitted to participate on a voluntary basis for The Academy of Industrials Arts l.l.c.
aka T.A.I.A. Inc. I, for myself and on behalf of my representatives, executors, administrators, assigns, heirs and next
of kin, hereby release, waive, discharge, covenant not to sue, and agree to indemnify and save and hold
harmless: City Electric Supply, The Academy of Industrial Arts aka T.A.I.A., The Southwest Community Development Center, and their respective officers, directors, Contractor-Ride-Along program participants, partners collectively and individually,
from any and all liability to me or my representatives, executors, administrators, assigns, heirs and next of kin, for any and all loss or damage, and any claim or demands therefore, on account of bodily injury, death or property
damage, whether caused by the negligence of the released parties or otherwise while I/my child am
participating in the event on July 26, 2025 at 215 Kedron Avenue Store #3 Folsom Pa. 19033. I agree before participating in any jobsite project or classroom performance tasks/projects with The Academy of Industrial Arts
Photographic and Media Release: Student/Volunteer and parent/guardian do hereby grant and convey unto
The Academy of Industrial Arts aka T.A.I.A., all right, title, and interest in any and all photographic images and
video or audio recordings made by The Academy of Industrial Arts aka T.A.I.A., during T.A.I.A.’S 's residential or
commercial repair program, classroom performance tasks/projects including but not limited to, any royalties,
proceeds, or other benefits derived from such photographs or recordings.
I expressly acknowledge that I have read and voluntarily sign this release and waiver of liability and indemnity
agreement, and agree that no oral representations, statements or inducements apart from the foregoing written agreement have been made.
